REGISTRATION INFORMATION

Fall Registration will be held

Tuesday, August 12-Friday, August 15

8:00 a.m. – 3:00 p.m.

At the time of registration, parents must have the following information. We cannot accept registration packets without ALL of the mandatory documentation:

Birth Certificate

To enter kindergarten a child must be 5 years of age on/or before September 15, 2008 (the district makes NO exceptions)

Immunization Record

Current immunizations must include:

5-DPT, 4-Polio, 2-MMR, 3-Hepatitis B, 2-Varicella (chicken pox)

Proof of Residency

Utility bill, lease agreement, real estate contract, etc. (we cannot accept a driver's license)

Current Phone Numbers

Phone numbers for home, work, two emergency contacts, and physician

Copy of Most Recent Report Card
